Position Summary: The case manager performs the primary functions of assessment, planning, facilitation, and advocacy, which are achieved through collaboration with the client and other healthcare professionals involved in the client's care.

Qualifications:

• Bachelor's Degree
• 2 years of acute hospital experience, including 1 year of supervisory experience
• Strong knowledge base or experience in specialty area assigned

Preferred:

• Master's Degree in Health care related field
