Case Manager, BHS- HHK


Job Details

Position Summary: Performs case management duties of assigned clients to provide timely, appropriate, and compliant care services. Coordinates and documents all behavioral health services and monitors the progress of clients throughout their treatment. Communicates relevant information effectively and efficiently to the appropriate staff members and relevant personnel when necessary.

Critival Tasks:

  • Ensures coordination of services for clients with low to moderate needs, including making referrals, following up on referrals, and the oversight of referred services.
  • Maintains regular contact with clients, service providers, medical providers, and other treatment team members.
  • Performs assessments and evaluations in order to identify clients' diagnoses and determine needs to develop an effective service/treatment plan.
  • Maintains accurate documentation of all services, clinical activities, contacts with clients, clients' families, and service providers.
  • Arranges and/or provides transportation to clients when necessary.
  • Ensures client charts are current and in compliance with State and Agency regulations.
  • Participates in treatment teams and staff trainings in order to ensure quality of services, and that the appropriate level of care is provided to all clients.
  • Performs other job-related activities as requested.

Required Qualifications:

  • Associates Degree in Counseling, Social Work or a closely related field of study required, plus 2 years of experience in Behavioral Health field or a closely related field of study.
  • Or
  • Bachelor's Degree in Counseling, Social Work or a closely related field of study preferred, plus 1 year of experience in Behavioral Health field or a closely related field of study.
  • Current and continuing BLS training and certifications preferred.
  • State of Arizona driver's license with no significant violations during the last 5 years.
  • Current State of Arizona Department of Public Safety Fingerprint Clearance card required.
  • Demonstrates ability to relate to diverse cultures and specifically the Gila River Community and/or other Native American cultures and community health services.
