Job Description:
The Social Service Worker provides psychiatric social work services for inpatient hospital clients for FMC and Psychiatric Health Services as part of a multidisciplinary team. Functions across a continuum of care by providing individual, family, and group therapy as directed and supervised by Psychiatrist.
- Collaborates with physicians and other members of the healthcare team to develop patient specific treatment plans.
- Monitors patients clinical progress throughout hospitalization through discharge.
- Provides ongoing assessment and consultation to meet patient and family needs.
- Assesses and provides educational instruction and materials to meet learning needs of patient and family members as related to each case and to healthcare providers relevant to particular cases.
- Completes psychosocial assessments.
- Completes multidisciplinary treatment plan.
- Assists with multidisciplinary assessment summary page for all discharges.
- Provides groups and individualized therapy as directed.
- Facilitates patient transfers to other psychiatric facilities.
- Coordinates with the psychiatric physician facilitation for judicial hearing needs.
- Coordinates post discharge needs.
QualificationsJob Qualifications:- Bachelors in Social Work with approval of Ohio Department of Mental Health permitted.
- Masters degree in Social Work preferred.
- Licensed Social Work (LSW) with practice directed and supervised by attending Psychiatrist(s) permitted.
- Licensed Independent Social Worker (LISW) preferred.
- A minimum of three years of experience or combination of experience and education.
Exerting up to 20-50 pounds of force occasionally, and/or up to 10-25 pounds of force frequently, and/or a negligible up to 10 pounds amount of force often.
