Mennonite Village in Albany Oregon is a not-for-profit CCRC facility that provides 24 hours/7 days a week care of varying levels. Lydias House is our Memory Loss Unit designed to care for elderly persons who have Alzheimers disease or related dementia. Due to the size of these units and the unique level of care; all staff may be required to provide some services primarily performed by others. Caregivers must combine compassion, activity focused care, and certain nursing skills to enhance the lives of Lydias house residents, enabling them to achieve success and increase independence.

Shifts/Hours available: Full Time/Part Time & On Call to fill in for call outs and vacations- Includes evenings and weekends. Shift differential for hours worked between 10pm & 6am.

ESSENTIAL FUNCTIONS:

- Be knowledgeable about residents service plans.

- Assist residents with activities of daily living (ADLs), bed making, diet and hydration, bathing, and personal laundry in addition to providing oversight to reduce falls & monitor resident behaviors.

- Monitor health status of residents and report changes to Team Leader or nurse.

- Respond appropriately to residents wearing safety devices.

- Use and model safety measures for residents and self, with assistance as needed, according to the safe handling policy by using gait belts, proper body mechanics, stand up lift, and other equipment as ordered.

- Practices standard precautions with every resident (i.e. use of gloves, proper hand washing).

- Assist with dietary tasks, such as meal and snack preparation, serving, set-up, clean up, and dish washing. Eat meals with residents as designated.

- Perform housekeeping tasks such as laundry, vacuuming, mopping, organizing, cleaning refrigerator, etc.

- Complete all electronic & paper documentation as required, such as resident meal intake, vital signs, etc., at the end of each shift.

- Maintain confidentiality of resident information at all times.

- Assist in planning, implementing and evaluating therapeutic programming. Plan and lead activities such as exercise, trips, crafts, parties, musical events, etc.

- Attend on & off campus in-services and staff meetings. Attend external educational sessions as directed.

- Attend resident care conferences as required.

- Provide direction to volunteers working in the neighborhood.

- Monitor supplies and keep management team aware of supply needs.

- Follow all Lydias House and Mennonite Village Policies.

- Protects and promotes residents rights as identified in the Mennonite Village Residents Rights.

- Know how to operate all equipment in Lydias House and report any problems.

- Regular attendance and punctuality are an essential function of the job.

- Abide by the standards and procedures of the Mennonite Village Compliance and Privacy Program and Values and Expectations Statement.

- Abides by the HIPAA policies and reports to a supervisor, breaches (or violations) of our HIPAA policies.

- Immediately reports identified hazards or hazardous conditions to a supervisor.

- Immediately reports adverse events and near misses to a supervisor.

- Attends mandatory training on HIPPA and compliance annually.

QUALIFICATIONS:

* Certified CNA in the State of Oregon preferred.

* Ability to describe the Social Model of care after appropriate education, and be willing to execute this model in Lydias House.

* Ability to work with and communicate effectively with the elderly.

* Must be able to handle giving personal care to residents, including the cleaning of bodily fluids and following proper safety procedures for clean-up.

* Capable of lifting 10-50 lbs., with occasional lifting of residents weighing over 100 lbs.

* Must have Heimlich Maneuver training.

* Must be positive, self-motivated, and have the ability to see tasks through to completion.

* Ability to safely move frequently during work day, including carrying, lifting, standing, sitting, bending reaching, and kneeling.

* Ability to push and/or pull wheelchairs and other equipment.

* Ability to be flexible, patient, creative, willing to learn and be motivated to enhance the lives of residents residing in Lydias House.

* Ability to focus on each resident as an individual.

* Sufficient English and Language skills to follow instruction from staff. Must be able to understand

* and respond properly to residents needs.

* Ability to accurately document activities in writing and in proper company software programs.

* Ability to work as a positive contributing member of a team.

* Ability to professionally handle emotional and behavioral changes, recognizing the physical progression of the diseases may increase resident needs (i.e. incontinency, verbal/physical outbursts) and support resident and families through these changes.
