We provide comprehensive primary and specialty health care and patient support services through a network of Centers surrounding Lake Okeechobee and across Florida s Treasure Coast. FCHC has locations in Clewiston, Moore Haven, Okeechobee, Fort Pierce, Port St. Lucie, Stuart, Indiantown, Pahokee and West Palm Beach. FCHC s Corporate Office is centrally located in West Palm Beach, Florida. FCHC has multiple staff members who speak Spanish and Creole, and translation is available for other languages as needed. FCHC has created a one-stop shop for patients providing Pediatric and Adult Primary Care, Dental, OB/GYN and Women s Health, Infectious Disease, Behavioral Health, Pharmacy, chronic disease education and care management, referral assistance, benefit enrollment assistance and coordination, and telehealth services. Position Summary: Provide and educate all aspects of patient care, including diagnosis, treatments and consultations. Work both inpatient and outpatient situations and can perform independently or as a part of treatment team. May conduct physicals, order tests and serve as a patient s primary healthcare provider.
This position requires the employee to travel to 2 other centers.
Role and Responsibilities:
- Help provide primary care to patients.
- Perform physical exams and patient observations.
- Must be proficient with medical instruments and equipment required by the work.
- Knowledge of computer based data management programs and information systems, as well as records and point of interview technology.
- Ability to communicate effectively, in verbal and written form, with retail and medical partners at various levels, patients, family members, physicians and representatives of the community.
- Record patient medical histories and symptoms.
- Create patient care plans and contribute to existing plans.
- Order, administer and analyze diagnostic test.
- Monitor and operate medical equipment.
- Diagnose health issues, detect changes in a patient s health and change treatment plans as necessary.
- Administer medicines and treatments.
- Evaluate responses to treatments and medications.
- Train patients and families on managing illness or injuries and disease prevention.
- Take active role to contribute to the improvement of office functioning.
- Ability to function independently with regards to most clinical decision making.
- Perform other related duties as required or assigned by the Medical Director
Qualifications And Education Requirements
- Master s degree from an accredited nursing program.
- Must have Florida RN Licensure and be board certified in Pediatrics (ANCC or PNCB).
- PNP authorization in Florida to practice in Pediatrics by the Board of Nurse Examiners.
- Successful completion of the National APRN Certification Examination Degree Program required for initial licensure.
- Current CPR (BLS) for the health care provider.
- Minimum one year experience as an APRN preferred.
- Knowledge of the scope of practice, legal replications to include statutes and regulations relevant to APRN.
- Advance knowledge of pharmacology, appropriate for care.
- Demonstrate written and verbal communications with the interdisciplinary team.
- Maintain harmonious, professional relationships with members of the care team, as well as patients and families.
- Knowledge of medication administration, side effects and calculation.
- Knowledge of infection control.
- Complete and maintain nursing CE requirements for each license renewal period.
