Quick Facts:

Under the guidance of the Director of Quality and Population Health, the Health Education Specialist will support the development and implementation of effective Chronic Disease Management at Eisner Health. In this pivotal role, you will develop health education programs that facilitate patient engagement, self-empowerment, and improved health outcomes. You will provide individualized health education and care planning. You will expand our Diabetes Empowerment Education Program, as well as develop new programs such as remote monitoring and "food as medicine" distribution. As a subject matter expert, you will also collaborate with interdisciplinary teams engaged in whole-person, patient-centered care, supporting seamless programming across the health system.


Duties Include:

1. Provide direct one-on-one health education services:

Engages individuals in critical conversations aimed at prevention, management of chronic diseases, and improvement of health outcomes.

Collaborates with clinical staff to manage care plans for patients enrolled in education classes.

2. Expand Chronic Disease Management Programming:

Develop and implement a peer-facilitated Diabetes Empowerment Education Program (DEEP) and expand reach across the health system.

Design and implement additional chronic disease management activities that engage patients in their care and emphasize patient empowerment, self-efficacy, and adherence to treatment plans, including remote monitoring.

Assess, develop, and implement food security programming tailored to patient needs.

Support cross-departmental programming as a Subject Matter Expert and provide training and support for other roles as appropriate.

3. Patient Education Materials and Cross-departmental training:

Develop and maintain diverse educational materials, ensuring relevance and accessibility for diverse patient populations.

Curate and create curricular materials on various health education topics for both individual and group settings, incorporating evidence-based practices.

4. Internal and External Collaboration:

Collaborate with healthcare professionals to ensure a coordinated and comprehensive approach to patient education.

Facilitate and document coordinated workflows across care teams for seamless delivery of programming.

Lead and participate in team meetings to support coordinated care.

Engage in community outreach initiatives to promote health education programs and raise awareness about chronic disease prevention and management.

5. Program Evaluation:

Implement assessment tools to regularly evaluate the effectiveness of education programs, making data-driven improvements.

Data analysis experience preferred.

Ability to develop reports tailored to various audiences required.

Proficient in MS Excel strongly preferred.


Requirements and Qualifications:

Master s degree in Public Health, Nursing, Nutrition, or related field OR Bachelor's degree with at least four years of experience in health education in a public health or primary health care organization.

Certification in Diabetes Empowerment Education Program (DEEP), preferred.

Certified Diabetes Educator (CDE), preferred.

Proven experience coordinating programs for patients living with chronic diseases.

Proven experience developing and/or curating curricular materials and evaluation.

FQHC or Community Health Center experience preferred.

Knowledge of benchmarking and national and state standards such as MCAS, STARs, HEDIS, and/or UDS preferred.

Ability to travel between clinics and community organizations ranging from the San Fernando Valley, DTLA, and Lynwood.

Ability to move up to 30 pounds, remain in a stationary position for extended periods, kneel, and crouch.

Bilingual/biliterate Spanish required.
