Job Description **$2,000 Sign on Bonus**

***Candidates must be external and have at least 12 months of Social Work experience to be eligible for the Sign-on Bonus.***

**Monday- Friday flexible hours**

**Service area for home care visits is Monroe County**

**General Summary:**

Provides social work services to clients to facilitate their understanding and resolution of problems related to environmental stress, physical illness and interpersonal conflicts that impede their treatment and recovery.

**Principle Duties and Responsibilities:**

1. Conducts psychosocial assessments with patients and/or significant others to determine and evaluate the following as appropriate:

-Support system and family relationships

-The meaning of physical/health status to the patient and family

-Emotional/mental status

-Financial impact of the illness to the patient and family

-Adjustment to illness/coping mechanisms utilized by patient and family

-The ability of patient and family to adjust to increased care needs of patient and the ability to meet those needs in the home

-Need for community resources, medication access etc

-Religious and cultural attitudes/beliefs related to the illness as appropriate

-Facilitates nursing home/extended care placement

-Facilitates and provides support regarding end-of-life issues

2. Counsels, educates and develops a discharge plan with patient and/or family as evidenced by documentation and case discussion. Coordinates plan with case manager.

3. Appropriately assesses needs and develops an age specific plan

-Neonate (<1 month)

-Infant (1 month-1year)

-Child (1 year-12 years)

-Adolescent (13-17 years)

-Adult (18-64 years)

-Geriatric (65+ years)

4. Maintains knowledge of age specific community resources for areas of service. Makes referrals to appropriate agencies to establish discharge plan and ensure continuity of care.

5. Promotes and maintains customer service focused care consistent with agency values.

-Serves as a role model for staff

-Works cooperatively with case mangers and supervisory staff to ensure patient care and service needs are effectively met.

6. Identifies patients and/or families who are at risk and in need of intervention. Examples may include:

-Those with pre-existing emotional/mental problems that impede functioning and interfere with medical care.

-Those who lack coping skills to deal with current situation.

-Those who by history have demonstrated difficulty adhering to medical treatment.

-Those who are in high risk categories by diagnosis or age, i.e., potential or history of substance abuse, domestic violence, sexual assault or child abuse.

7. Provides counseling to patients and significant others in relation to their medical-social issues, i.e., adjustment to illness and disability. Facilitates referrals for ongoing counseling as needed. Documents goal-oriented objectives and results achieved from counseling in a clear and conscious manner.

8. Assesses and coordinates financial referrals and counsels patients and their families informing patients and families of various assistance programs and community resources.

9. Ensure accuracy of clinical documentation.

-Participates in comprehensive chart review.

-Ensures documentation of assessments, re-visits, and follow-up interventions are performed on a timely basis.

10. Demonstrates the ability to prioritize complex caseload to effectively meet patient and staff needs and maintain productivity expectations

11. Participates in agency team and case conferences as appropriate to identify social problems, their severity and their impact on patients health as well as the patient and caregivers strengths and weaknesses and their ability to handle stressors and discussing alternate methods of resolving situation.

12. Maintains confidentiality of all agency and patient records.

13. Performs other job related duties as assigned.

Job Requirements

1. A masters degree in social work.

2. Social Work licensure in the State of Michigan as a LMSW or LLMSW.

3. One year social work experience in a health care setting required, home care experience preferred.

4. Ability to work effectively on an interdisciplinary health care team.

5. Must have reliable transportation to entire service area.

6. Maintains knowledge of community resources and referrals to all service areas.

7. Must be able to move about the facilities and between workstations and work sites per established productivity levels.

8. Must be able to understand directions and communicate and respond to inquiries; requires effective interpersonal skills.

9. Must be able to tolerate exposure to dust, temperature changes.

10. Must be able to occasionally move, lift, and/or carry medium weight objects or materials.

11. Must be able to use computerized documentation system and laptop computer.
