Job Details

• Leads group and individual programs both structured and impromptu in activity spaces, common areas, and in resident's rooms. Assists Activity/Wellness Manager in the designing, planning and preparation of various activity programs to meet the needs of assigned residents. Assists Activity/Wellness Manager with Calendar and Newsletter Completion.
• Assesses resident's needs and limitations on an on-going basis for program planning, written assessments, MDS, and care planning. Consistently maintains proper documentation including daily participation records, assessments, and progress notes (quarterly, initial, annual) for LTC and six-month review for ALF, plan of care, and MDS.
• Works collaboratively with other team members, volunteers, families, and community groups.
• Generates activity supply lists, maintain supplies and equipment, keeps work areas in a safe and functional order.
