General Summary of Position
Part of a team responsible for creating, maintaining and leading the clinical outreach program as part of the hospital's mission to promote the public health of our regional community. An ongoing schedule of screening and educational activities that is tied to the strategic plan of the hospital will be developed and implemented. The incumbent will also be part of a team providing transitional care case management to patients of MSMHC experiencing challenges in chronic disease self management and access to the healthcare system in the community.
Primary Duties and Responsibilities
Perform Health Risk Assessments and health screenings for disease like diabetes and high blood pressure.Administers injections for vaccinations.Provide patient information and obtain appropriate consents for screenings and procedures performed.Contributes to the successful implementation of the Community Health Needs Assessment plan and data collection needed for annual community benefit reporting.Facilitate partnerships in the community with schools, faith-based organizations, community groups and county government.Teaches evidence based classes and programs.Responsible for staffing community outreach events.Carries a caseload of patients at high risk of readmission in coordination with the RN Transitional Care Navigator.Perform all other related tasks as assigned.
Minimum Qualifications
Education- Associate's degree Nursing required
- Bachelor's degree in Nursing preferred
Experience- 3-4 years Working in the health care industry, preferably acute care required
Licenses and Certifications- RN - Registered Nurse - State Licensure and/or Compact State Licensure in Maryland required
Knowledge, Skills, and Abilities- Ability to comprehend and follow the MedStar Southern Maryland Hospital Center Associate Handbook.
- Ability to comprehend and follow the policies and procedures for MedStar Southern Maryland Hospital Center.
- Ability to read, write and speak or communicate in English to successfully accomplish the essential duties of the position.
- Ability to demonstrate ethical behavior that supports the hospital's mission, values and commitment to compliance with all federal, state and regulatory laws. Inspires trust and exhibits honesty and integrity within the scope of daily activities.
- Exhibits professionalism, courtesy and excellent customer service, while interacting with patients, guests and co-workers.
