Job Details
Leads group and individual programs both structured and impromptu in activity spaces, common areas, and in resident's rooms. Assists Activity/Wellness Manager in the designing, planning and preparation of various activity programs to meet the needs of assigned residents. Assists Activity/Wellness Manager with Calendar and Newsletter Completion.
Assesses resident's needs and limitations on an on-going basis for program planning, written assessments, MDS, and care planning. Consistently maintains proper documentation including daily participation records, assessments, and progress notes (quarterly, initial, annual) for LTC and six-month review for ALF, plan of care, and MDS.
Works collaboratively with other team members, volunteers, families, and community groups.
Generates activity supply lists, maintain supplies and equipment, keeps work areas in a safe and functional order.
