Job Details

Coordinate and manage care for patients by assisting patients in navigating their care across the continuum to include making annual wellness visit (AWV) and transition of care outreach phone calls. Identify patients who are eligible for a Medicare Annual Wellness Visit and educate the patients on the importance of these visits. Identify at-risk patient populations who have been admitted to the hospital or other in-patient facility, or had a recent ER visit, and communicate with those patients in order to answer any questions and schedule a follow up visit within 7 days of discharge, educating them on the importance of following up with their primary care provider (PCP). Must possess excellent communication and time management skills.

Education: High School Diploma or GED required. Graduate of a Registered Nursing program, Practical Nursing program, or CAAHEP or ABHES accredited medical assisting program preferred or a combination of education and experience will be considered.

Certification: A current CMA or RMA credential through one of the following: American Association of Medical Assistants, American Registry of Medical Assistants, American Medical Technologists, National Center for Competency Testing, or National Health Career Association.

Licensure for LPN: Current valid NC LPN License.
2 years experience as an LPN or medical assistant is preferred. BLS Certification required
