Description

This position is part of the clinical treatment team within Fond du Lac County Human Services. Substance Use Disorder (SUD) counselors are responsible for evaluating/assessing, diagnosing, and treating individuals with substance use disorders. SUD counselors are required to participate in clinical staffing, treatment planning, crisis intervention services, community prevention efforts, and provide consultation within the agency, community partners, and pubic.
Examples of Duties

Essential Duties:

  • Provides comprehensive assessments, screening, and documentation of new client intakes.
  • Maintains professional documentation of ongoing clients.
  • Prepares individualized treatment plans and completes required reviews.
  • Maintains professional and confidential relationships with clients.
  • Conducts individual and group therapies in the outpatient clinic and inpatient psychiatric unit.
  • Participates and attends required clinical staffing's and outpatient meetings.
  • Provides and participates in prevention efforts within Fond du Lac County.
  • Provides CCS service facilitation for individuals diagnosed with a SUD.
  • Provides crisis intervention services to clients in in need of immediate assessment and/or referral.
  • Works collaboratively with outside Departments to include: Department of Social Services, Probation and Parole, Schools, Jail, Department of Transportation, Residential Facilities, Courts, etc.
  • Participates in coordination and care of clients involved in the Drug Court Treatment Program.
  • Provides OWI assessments, develops Driver Safety Plans, and provides case management to clients in need of treatment services.
  • Performs other duties as assigned.
Typical Qualifications

Qualifications/Requirements:
  • Licensed SUD counselor in the State of Wisconsin required (SAC-IT, SAC, CSAC).
  • Bachelor's degree in Human Services or related field is preferred.
  • Valid Wisconsin driver's license and automobile insurance.
