Responsibilities:
Provide services to address activities of daily living, including the management of symptoms associated with psychiatric, physical, and/or substance use disorders per program standards and the 8 Dimensions of WellnessComplete services consistent with the requirements outlined in the expectations worksheet and program manual with the supporting documentation completed in a timely manner (within 72 hours).Maintain all assigned client medical records to program standardsProvide referrals, advocacy, information and education to meet individual needs of clientsProvide appropriate risk assessments and facilitate access to needed interventions as indicatedProvide transportation for clients as neededMonitor client hospitalization and participate in discharge planning and follow per program standardsAvailability to provide services in evenings as assignedAttend court or other community based meetings as neededMaintain communication with referral sources and/or community stakeholders regarding client participation and progress in treatment, as neededAssist counselors with outreach to help ambivalent clients re-engage in treatment.Assist with client UA's as neededOther clinical needs as necessary or assigned, including but not limited to, providing education to ADA clients in group setting
