This is a part time 20hr/week position

Social Services Assistant duties include but are not limited to:

  • Evaluating new residents upon admission with social history assessments, BIMS, communication.
  • Compile information and identify resident concerns/needs, goals as relates to social service needs.
  • Assist the Social Services Director in implementing residents' social service care plan.
  • Assist resident in financial and legal matters.
  • Assist with Advanced Directives and documents such as healthcare and financial POA.
  • Set up and attend care conferences with new residents as well as long-term residents at least on a quarterly basis. This would include inviting case managers from managed care insurance plans as well as family members/friends.
  • Completing PASRR screens upon admission and as needed.
  • with training become familiar with MDS assessments and using PointClickCare (PCC) electronic health records.
  • Keeping track and resolving grievances on an ongoing basis.
  • Liaison between facility and home health care agencies.
  • Help set up services to be used when discharged from the facility. This includes Meals on Wheels, home health, PT/OT, caregivers, etc.
  • Completing NOMNCs and ABNs for residents and their changes in payor sources.identifying areas of concern and coordinating services with the interdisciplinary team as well as contacting community resources as needed.
  • Assist Social Services Director in ensuring medically related emotional and social needs of the resident are met and maintained on an individual basis.

Requirements:
  • High school diploma.
  • Experience working with the elderly and dementia.
  • Experince using Point Click Care a plus but not required
