The Position:

We're looking for an experienced and passionate Clinical Liaison (RN) to serve as contact between referral sources, Ohana Pacific Health (OPH) communities, and OPH admission department by conducting referral reviews, clearances, and admission processing for OPH communities.


Responsibilities include:

  • Coordinates and assists with admissions to ensure high resident census.
  • Interprets OPH policies and governmental regulations, related to admissions and on-going stay, to residents and responsible parties.
  • Manages OPH admissions process for all referrals.
  • Daily communication with assigned communities to monitor availability of beds, planned admissions, discharges and any barriers or requirements for admissions.
  • Communicate referral status to admissions team and disseminate necessary documents for admission.


Requirements:

  • Graduate of an accredited school of nursing.
  • Current license as a Registered Nurse in the State of Hawaii required.
  • One (1) year experience coordinating admissions in a healthcare facility.
  • Experience with Medicaid eligibility.
  • Experience in post-acute healthcare setting, clinical assessments, and financial requirements preferred.
