Job Details

The Director of Community Health is responsible for the leading the outreach, coordination and delivery of Health Education and Empowerment Programs and overall outreach at locations throughout the community.

Essential Functions / Job Duties:

  • Oversees and supervises community health grants and all of their respective deliverables.
  • Manages and coordinates community health programs and staffing, grant management, budgeting and reporting.
  • Manages Delivery of all community outreach and Community Health Worker staff.
  • Ensures CHW staff attend specialized trainings including HIPPA, MotivationalInterviewing,MentalHealthFirst Aid, Racial Equity, DSMP, CDSMP, MOD and others, etc.
  • Ensures CHW team use community health facilitation techniques appropriately and effectively.
  • Ensures CHW team keep daily and weekly up to date records.
  • Ensures CHW team notify Director if need to be absent in a timely manner.
  • Ensures CHW team keep in touch with Director to report attendance, discuss, and resolve any problems.
  • Establishes implements, maintains community partnerships, and serve as a liaison between YMCA and community sites and partners.
  • Conducts field visits, outreach and serve as resource to staff, participants and partners.
  • Coordinates and participates in community site events.
  • Recruits program participants including conducting presentations and distributing printed materials.
  • Identifies community leaders and maintain a community contact list.
  • Maintains records of registered participants and programs sites.
  • Updates Director/Supervisor on a weekly basis with program updates.
  • Establishes trusting relationships with participants while providing general support and encouragement.
  • Engages and motivates participants to be active participants in their health.
  • Provides ongoing follow-up, motivational interviewing and goal setting with participants.
  • Utilizes motivational interviewing and teaches approaches to ensure that participants understand their health issues and how to manage them.
  • Follows-up with participants if needed with calls and visits to other settings where participants can be found.
  • Helps participants develop heath management plans, goals, and follows-up on health managementcareplans with participants and providers.
  • Provides referrals for services to community agencies as appropriate.
  • Helps participants connect with transportation resources however transporting participants is strictly prohibited.
  • Acts as resource to community members about community resources appropriate to needs of community members.
  • Assesses participant s' interest in evidenced-based programming and assists participants in enrolling in such programming.
  • Provides appropriate referrals regarding community resources to evidence-based programs such as A Matter of Balance, Chronic Disease Self-Management, Diabetes Self-Management, Enhance Fitness, YMCA Diabetes Prevention Program and others.
  • Participates in ongoing health trainings.
  • Organizes quarterly staff meetings in partnership with colleagues.
  • Completes assignments by designated deadline/ timeline.
  • Keeps abreast of health empowerment opportunities and resources for participants
  • Develops working understanding of health disparity and resources to overcome.
  • Adheres to all risk management and safety protocols.
  • Translates medical jargon into easily understandable lay language.
  • Understands the importance and purpose of fidelity (following the program protocols).
  • Assumes other duties and projects as needed and as assigned.

Qualifications, Skills and Abilities/ Position Requirements:

Education and Experience:

  • Bachelor's degree in Social Work, Psychology, Public Health or related field, or equivalent years of direct experience in Case Management. Master's degree preferred.
  • At least two (2) years' case management or community work experience.
  • Excellent interpersonal skills; able to communicate effectively with a diverse group of employees, volunteers and program participants.
  • Excellent communications skills; able to communicate with individuals of the targeted audience (at about 10th grade level).
  • Ability to relate and connect to people andworkwithadiversepopulation;understandandpractice YMCA Character developmentvalues of Caring, Honesty, Respect and Responsibility.
  • Ability to translate medical jargon into easily understandable lay language.
  • Certification as a Certified Community Health Worker is highly desirable.


Certificates and Licenses:

Current CPR and First Aid certificates by time ofemploymentorwithinninety(90)daysfromthedateof hire.

Other Skills and Abilities:

  • Teaching, training and public speaking experience preferred.
  • Ability to work well with others understanding and use of Motivational Interviewing skills.
  • Able and willing to work nights and weekends.
  • Has reliable transportation.
  • Ability to establish and maintain positive and collaborative working relationships with participants, staff and community partners.
  • Possesses life experience resulting in empathy to the needs and abilities of persons with chronic health conditions or other health issues, vulnerable populations, immigrant, low-income, special needs, underserved and undocumented communities.
  • Possesses critical thinking and evaluative skills.
  • Knowledge of basic health information! interventions and resources.
  • Bilingual Spanish and/or Creole preferred.
