Job Details

Summary of Responsibilities:

Patient Care Coordinator is responsible for the planning, organizing, and staffing of all Prevention and HIV testing programs for the Client Services Department.


Details of Education and Skills:

Bachelor s degree in social sciences from an accredited learning institution, with a minimum of three (3) years experience in HIV/AIDS. Or Five (5) years or more experience & training in HIV/AIDS may be substituted for a degree.

Minimum 2 years experience in a supervisory role.

Experience coordinating & working outreach events and LGBT community.

Excellent communication (oral & written) skills.

Must be proficient with computers (Microsoft Word and Electronic Health Records) EHR

Ability to work in team environment successfully.

Special Requirement:

HIV 104 Training Certificate, HIV 501 Training Certificate.

Bilingual is a plus (English / Spanish or English Creole)

Current CPR certification.
