Job Details

POSITION SUMMARY
This person will assist both outpatient and inpatients in coordinating healthcare needs, with community healthcare and psych/social resources. During your average day, this person will:

  • Skillfully conduct screenings and/or full psych/social assessments, and recommend/coordinate further care planning and intervention necessary.
  • Using exceptional relational skills as well as outstanding follow through abilities, work in cooperation with the multi-disciplinary team to serve diverse populations as needed.
  • Conduct mental health evaluations, and build safe discharge plans out of the emergency department and the Med-Surge floor, using a high level of competency in understanding mental health, substance use, homelessness, and other barriers to care.
  • Serve as the liaison between patient discharge and follow-up with patient's primary care provider in the community.
  • Address barriers to patient's achieving optimal healthcare, and problem-solve care plans as appropriate.
  • Participate in interdisciplinary medical rounds and case reviews in the areas of Med-Surge, Emergency Departments, Medical Ambulatory Clinic (oncology), Family Birthplace, and other departments as assigned.

Education
Masters of Social Work (required)

Licensure
Licensed in the state WA as an LSWAIC or LICSW (required)

Shifts
Rotating shifts, day and evening hours, weekends, holidays
