ResiHealth provides on-site, preventative primary care at active senior living communities, focusing on supportive management of chronic health conditions including diabetes, COPD, hypertension, and obesity.

ABOUT THE ROLE:


The Senior Nurse Practitioner (NP) serves as an integral part of the care team and is responsible for providing direct patient-centered care in the ResiHealth medical clinics and affiliated residences. The NP works autonomously, providing comprehensive care to patients. Additionally, the NP will also work closely with clinical and operational staff for program development and practice optimization.


The NP conducts age-appropriate assessments of patients, including medical history and physical examination, diagnosis and treatment, wellness and disease prevention consistent with ResiHealth policies, procedures, and protocols. The NP will participate in the development of ResiHealth clinical operations programming including but not limited to establishing a state of the art Remote Patient Monitoring ( RPM ) treatment plan, health education and promotion, and care coordination managing physician referrals. The NP establishes and maintains accurate clinical documentation and communication with the care team and stakeholders to ensure operational efficiency

and effectiveness and to promote continuity of care.


The Nurse Practitioner must demonstrate the ability to function both independently and in collaboration with other health care professionals. They will routinely consult with the manager, physician, and medical director to ensure compliance with guidelines. This position may require participation in risk and quality management programs, clinical meetings and other meetings. They will adhere to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures.


An important responsibility of the Senior Nurse Practitioner will be to optimize the care delivery model(s) and workflows for ResiHealth s on site clinics and provide critical feedback to the leadership team for strategic product-market fit development.


ESSENTIAL JOB DUTIES/RESPONSIBILITIES:


Provides medical care for patients with complex, chronic and/or acute conditions

Participates in acute and chronic care management of patients, specializing in assessment and documentation of high-risk conditions

Performs history and physical assessments

Performs minor procedures after established privileges, training and proctoring is completed for the procedures

Orders and manages follow up diagnostic testing (laboratory and radiographic findings)

Prescribes medications per ResiHealth standards of care

Provides patient education and health promotion

Provides triage throughout all points of access to care

Participates in population health management

Participates in synchronous and asynchronous telehealth care

As an independent NP, assess acute and non-acute clinical problems. Perform and document Annual Wellness Visits. Develop, document, and implement patient care plans in response to clinical care metrics. Assist in the development of the treatment plan in addition to providing appropriate patient/family/significant other counseling and education when appropriate.

Identify and assign patient remote physiological monitoring (RPM) point solutions based on HRA and AWV information. Monitor RPM data feed, alerts, and alarms to analyze and address clinical interventions based on Telehealth KPIs & patient engagement insights in conjunction with the medical director.

Plan patient care based on in-depth knowledge of the specific patient population and/or protocol, anticipating and identifying physiological and/ or psychological problems commonly encountered including the consideration of the patient s cultural background, level of understanding, personality and support systems. Serves as patient advocate.

Patient management includes the following:

Places orders and interprets appropriate laboratory and diagnostic

studies.

Places orders of appropriate medication and treatments.

Ability to create a comprehensive and coordinated plan of care that incorporates information from medical records, the patients and their families, as well as Primary Care Physicians and Specialists.

Ability to use Electronic Medical Record (EMR) system to document patient encounters.

Participates in patient care rounds and conferences. Communicates patient management strategies to members of the patient care team.

Collaborates with members of the multidisciplinary team to ensure that patient management strategies are successful in meeting patient care needs.

Recognizes situations which require the immediate attention of a

physician and initiates emergency contact procedures.


Other Responsibilities may include:


Collaborates with cross-discipline team to evaluate current workflows of healthcare delivery to identify and implement new and higher quality practice patterns as appropriate.

Contributes to strategy and design on Care Delivery Framework.

Builds collaborative relationships with patients to maximize their ability to make informed decisions.

Prescribes medication to patients based on State of practice.

Performs other responsibilities as assigned and modified at manager s discretion.


KNOWLEDGE, SKILLS AND ABILITIES:


Experience in a leadership capacity

Experience with geriatric care

Experience working with medicare and medicaid population

Demonstrated record of consistently achieving clinical performance metrics in current role

Strong Critical Thinking and problem-solving skills

Excellent verbal and written communication and interpersonal skills

Must be caring and empathic and have great listening skills

Demonstrated proficiency in clinical assessment, diagnosis, planning, implementation, documentation, and evaluation regarding the needs and care of chronic disease

Deals with confidential information and/or issues using discretion and judgment

Excellent organizational skills and attention to detail.

Intermediate computer skills and general knowledge of Microsoft Office Suite


EDUCATION AND EXPERIENCE CRITERIA:


Current Nurse Practitioner Certification in the State of OH

AHA BLS CPR required

Demonstration of clinical competency in the Advanced Practice Provider role

Board certification by AANP or ANCC is preferred but may be required for certain States

Current DEA number from the DEA for schedule II-V controlled substances may be required based on State of practice

5+ years of clinical experience preferred

Demonstrated expertise in treating and managing Medicare and Medicare Advantage populations with multiple comorbidities

Virtual medical treatment and management of high-risk population

Demonstration of qualities of leadership and sound clinical judgment. Ability to effectively communicate with patients, fellow co-workers and other members of the health care team. Ability to effectively interpret, implement and support ResiHealth policies, regulations, philosophy and

objectives
