JOB DESCRIPTION - CLINICAL NURSE MANAGER



JOB SUMMARY:

The Clinical Nurse Manager has the responsibility/accountability for leading and managing the clinical team. Leading by example while managing the clinical staff in the daily care of patients and the documentation of their care in the EMR as well any paper documentation. Uses skills in planning, organizing, delegating, and supervising the daily tasks of the clinical team. Work effectively with physicians, mid-levels, staff, patients, public and external agencies. Ability to take initiative and to exercise independent judgment, decision-making and problem-solving expertise within the guidelines of the practice and the providers.


REPORTS TO: Physician Owner

MANAGES: Provider Nurses, CCM Nurses, Clinical Specialist-Quality


ESSENTIAL FUNCTIONS:

  1. Leadership Skills: Leads employees to accomplish all job objectives; inspires confidence and motivation; clearly defines expectations and maintains personal effectiveness under pressure.
  2. Collaborate with the management team to ensure evidenced based health care standards are understood, organized, and performed by all clinical staff.
  3. Able to perform and train all clinical process and procedures. Directly, or through delegation, monitor patient care for quality and chart review for appropriate clinical documentation.
  4. Communicates effectively with providers, patients, staff in a courteous and productive manor.
  5. Adjust priorities as needed for the appropriate situation, keeping the clinic workflow functioning appropriately and on time.
  6. Report to the physician owner and works together with the practice manager to meet organizational goals. Contributes to meetings and team collaborations.
  7. Perform annual clinical staff reviews using the IFC performance tools.
  8. Support the clinical needs of the Fitness program as needed.
  9. Ensure that New Patient Packets are reviewed for completeness and work with appropriate provider nurse to get new patient information entered in the system.
  10. Lead the CCM program, including staffing, work, productivity, and quality expectations. Ensure that CCM activities are billed as appropriate in a timely manner.
  11. Manage relationship with and interface to the onsite lab vendor.


2.Clinical Inventory Management: Work with clinical staff to ensure supplies and vaccines are organized, available and meet clinical standards for patient care.

  1. Organize a stocking process for the clinical staff to keep the examination rooms prepared for patient care. Ensuring the exam room have organized and have adequate supplies.
  2. Ensure all supplies and exam rooms meet clinical standards.
  3. Ensure that vaccines supplies are monitored and ordered in time to meet anticipated needs.
  4. Clinical Supervision: Ensure organization of clinical workflow. Working with the administrative team for a unified practice environment.
  5. Attend daily huddle and evaluate workflow process, lead ideas, and implement positive leadership.
  6. Evaluate workflow through the day keeping the providers and staff organized and on time.
  7. Monitor clinical effectiveness and efficiency, and lead positive change where needed.
  8. Work to ensure appropriate staffing coverage. Ensure staff are at work during normal scheduled hours and any infractions or absence, or PTO are documented. Review and approve all clinical staff PTO requests to ensure appropriate coverage while also ensuring that staff is taking an appropriate minimum of PTO as part of our work-life balance goals.
  9. Organize and monitor prescription refills, referrals, phone messaging, faxing, and additional tasks outside of direct patient care.
  10. Attend and/or lead weekly/monthly meetings and contribute to the practice clinical workflow, practice and personnel growth and revenue capture.
  11. Administrative team collaboration, support provider, mid-level and coordinators in planning, organizing and process improvement.
  12. Oversee EMR use and contribute to the development process to ensure quality documentation and efficient workflows to include vitals, health maintenance, clinical elements orders, referrals, progress notes, patient messaging and in conjunction with Group Medical Visits (GMV) coordination and high-risk care coordination.
  13. Liaison between the providers and staff during the clinic hours to eliminate interruptions and distractions during patient care.

3 Environmental Care: Ensure safety precautions for clinical standards, CLIA and OSHA guidelines. Set safety and emergency guidelines.

  1. Oversee all clinical standards, precautions are trained and in daily practice.
  2. Safety guidelines are available, trained and followed.
  3. Emergency guidelines are trained and followed. Including Police, fire, weather, and any related emergencies.
  4. Ensure that Spore Test and Auto Clave Cleaning are completed on an appropriate schedule.
  5. Work with patients, directly or through delegation, to address Medical Assistance Patient Support programs that may be available to the patients.
  6. Complete required State reporting of in-house positive Flu and Covid tests.


QUALIFICATIONS:

  • High School Diploma or equivalent required
  • LPN or RN license and equivalent work history required.
  • Possesses good grammatical and business communication skills.
  • Knowledge of standard office machinery including computer systems and applications.
  • Proven ability to work in a clinical leadership role independently with minimal day to day supervision.
  • Two years medical family care office experience preferred. Clinical management experience required.
  • Knowledge required of EMR, progress notes, ICD-10 and CPT coding and how it affects reimbursement from third party payors.
  • Able to attend to office needs before or after regular office hours as needed.
  • CPR certified
  • Ability to lift, carry, push, and pull up to 50 pounds.
