**Position Summary**
Serves as a point of entry and resource for potential clients and referral partners, both internally and externally. Makes recommendations for solutions, connects clients with resources and assists individuals in navigating the healthcare system; finding solutions that holistically meet their needs. Provides support for all internal and external customers and facilitates the intake process for Home Health and Hospice Services.
**Essential Duties and Responsibilities**
The following statements describe the principal activities of this position identified and shall not be construed as a detailed exposition of all the duties that may be inherent in this position.
* Responds to referral inquiries received by phone, website, online inquiry, fax or portal;
* Assists existing or potential clients in identifying the services that will best meet their needs;
* Accurately processes received information to complete the referral to be sent to respective region/division for admission;
* Contacts physicians for orders and other information when necessary in order to complete the referral for Home Health or Hospice admissions;
* Maintains Physicians and facilities maintenance in HCHB;
* Complete documentation in HealthMEDX, HCHB and other software systems as necessary
* Schedules visits for initial admission;
* Assists in completing and/or obtaining necessary paperwork to facilitate admission;
* De- escalates issues with customers and identifies permanent resolution to issues;
* Escalates customer issues that require additional customer care to the right contact to provide resolution;
**Education and Experience Requirements**
Education:
* High school diploma or equivalent required.
* Some college or post-secondary education preferred.
Experience:
* Minimum one (1) to three (3) years experience in Customer Service, Sales, Data Entry/Clerical Experience, Healthcare Delivery, Administration or Management, Patient Advocacy or similar field preferred
