**New Perspective is seeking Caregivers and Caregiver Med Passers to travel to our assisted living and memory care communities throughout the Twin Cities. Reliable transportation is important.**

Through the direction of Community clinical leaders, the **Resource Caregiver Med Passer** provides residents with timely and quality care services, to include, but not limited to assistance with activities of daily living, medication passing, and managing behavioral expressions. The resource caregiver med passer may be assigned to work at multiple communities in the Twin Cities on an as needed basis. This position may work in the assisted living neighborhood, memory care neighborhood, or both. **RESPONSIBILITIES**

* Executes daily, weekly, and monthly caregiver task assignments including helping residents with activities of daily living, such as grooming, assistance with eating, and other care giving services to residents

* Maintains and monitors resident vital signs and weight in accordance with physician orders and/or nursing directives

* Assists in meal preparation, resident eating, and related services

* Collaborates with team members to create and maintain individualized resident services records

* Works with team members, residents, and families to support initiatives for the Life Engagement, and memory care programs, as applicable

* Ensures nursing leadership is timely notified of actual or potential resident change in condition or other care-related issues

* Attend and participate in all required trainings, team meetings, town hall meetings, online learning resources, and others as required

**Medication Passing:**

* Upon delegation by a registered nurse and/or certification as a med passer, timely administers medications in accordance with Company medication administration policies and procedures and applicable state laws

**QUALIFICATIONS**

* High school diploma/GED, or as required by applicable state laws

* At least one year of caregiver experience working with older adults in a senior living, long-term care, home health or other health care setting preferred

* Experience passing medications or ability to be trained to pass medications

* Ability to work in a team environment with strong communication and interpersonal skills

* Ability to anticipate and solve problem

* Computer skills and ability to use a variety of electronic devices

* Ability to communicate effectively verbally and in writing using the English language

* Ability to handle multiple tasks simultaneously

* Ability to work a flexible schedule, including weekends and holidays
