Nurse Practitioner Job Description
Working collaboratively as a key member of the medical sta , the NP plays a critical role in managing the medical needs of Skilled Nursing Facility patients and Long-Term Care residents. Primary Care Select NPs build strong working relationships with Facility Administrators, Directors of Nursing, and other sta within their designated facility, functioning as the primary Provider point of contact for acute care and other medical needs.

Nurse Practitioner Responsibilities
- Provide Post-Acute medical services to Skilled Nursing Facility patients recently discharged from local hospitals. Oversee the rehabilitation process and plan and prepare patients and families for eventual patient discharge.
- Provide primary, urgent, and preventive care to Long-Term Care residents.
- May provide similar care to local Assisted Living Facility residents.
- Verify admission orders and conduct initial assessments on new Skilled patients and newly admitted Long-Term residents.
- Conduct Physical exams, medication reviews, and medical screenings.
- Collaborate with the Facility Medical Director, attending physicians, and nursing sta to deliver the highest level of care possible to all patients in the Nursing Facility.
- Communicate with patients, residents, and family members regarding changes in clinical status, treatment options, and prognosis.
Minimum Quali cations
Valid Nurse Practitioner and RN Licensure in the state of Alabama
Nurse Practitioner National Board Certi cation
Clinical training or background in adult, internal, family, and/or geriatric medicine
Vaccination against COVID-19 is required for all new hires unless the individual qualifies for a remote, religious, or medical exemption.
Preferred Quali cations
Experience in internal medicine or gerontology
Excellent administrative, organizational, and verbal skills
Ability and desire to work independently
Detail oriented
Dependable and reliable
