Responsibilities
- Conduct assessments to identify individual needs and develop care plans
- Collaborate with healthcare providers, social workers, and other professionals to ensure comprehensive care
- Monitor patient progress and adjust care plans as needed
- Provide counseling and support to individuals facing mental health challenges or addiction
- Maintain accurate and confidential patient records in compliance with HIPAA regulations
- Advocate for patients' rights and access to appropriate services

Qualifications
- Provisional MHRT-C (Required)
- 1 year of case management experience (preferred)
- Intermediate level computer experience
- Valid Driver License, auto insurance and reliable vehicle (Required)
