SCHEDULE 0800-1700 MONDAY-FRIDAY

JOB SUMMARY

Responsible for providing psycho-social support, short-term counseling and necessary referral for patients and families. Demonstrates effective communication skills employing empathy and compassion when working with the patient/families in the provision of social services. Able to work with age specific populations according to the setting. Provides case coordination, referral to community resources and consultation. Coordinates patient placement to nursing home, assisted living, mental health facilities, etc. In some cases may provide supervision for BS level social worker.

Minimum knowledge, skills, and abilities:

• Licensed Clinical Social Worker or an equivalent combination of BSW and Master's level education in a clinical mental health field
• Prefer previous health care experience.
• Knowledge of Medicare and insurance industry.
• Knowledge of Joint Commission requirements.
• Ability to read and communicate effectively in English.
• Strong written and verbal skills.
• Basic computer knowledge.
• Must demonstrate critical thinking skills and decision-making ability required for management responsibilities.
• Ability to communicate with tact and professionalism.
• Ability to work independently and to communicate effectively.
