Job Details
Position: RN Discharge Planner - inpatient
Location: Washington, DC
Day-to-Day Responsibilities:
- Enjoy a Monday to Friday schedule, no weekends or holidays required (occasional weekends, 2-3 per year).
- Participate in daily rounds at a consistent time (around 10:30 AM) with the same dedicated team.
- Collaborate on team-based assignments including providers, Utilization Management (UM), Nutrition, and Social Work.
- Coordinate care for discharge planning, ensuring the smooth transition of our pediatric patients.
- Cover a caseload of up to 25 beds per day, though typically less.
- Receive thorough training and orientation lasting 6-8 weeks, tailored to your experience level.
- Benefit from ongoing support with weekly check-ins, especially during the initial two months.
- Experience shadowing opportunities, rotating with each team to gain comprehensive insight.
Training and Experience:
- Complete a 6-month probationary period (Paid) with a detailed orientation and training program.
- Ideal candidates will possess a strong clinical background, preferably with case management experience.
- RN, BSN required; MSN preferred.
- Pediatric experience highly desired.
- Must exhibit confidence, strong communication, and organizational skills.
Certifications Needed:
- RN, BSN required; MSN preferred.
- Previous case management experience preferred.
- Proficiency with Cerner is advantageous.
Compensation:
- Competitive salary range of $90,000 - $95,000 annually, based on nursing experience.
