Registered Nurse | Case Manager | Remote | Contract
Walker Healthforce is seeking a RN Case Manager for a healthcare client located remotely. This is a contract opportunity.

JOB DESCRIPTION - Lead a multidisciplinary team in delivering holistic, person-centric care management for a diverse health plan population.
- Act as the primary contact for members, caregivers, and providers through various communication channels.
- Utilize the case management process to assess, develop, implement, monitor, and evaluate care plans across the care continuum.
- Coordinate with Medical and Behavioral Health Social Workers, Registered Dietitians, Pharmacists, and others.
- Assess members' health, psychosocial needs, and support systems, developing individualized care plans.
- Arrange necessary resources and coordinate care delivery among support systems, providers, and community agencies.
- Advocate for members and provide education on health literacy, self-management, medication plans, and nutrition.
JOB REQUIREMENTS - 3+ years of clinical nursing experience in a clinical, acute/post-acute care, and community setting
- 1+ years of case management experience in a managed care setting
- Compact RN License in the state you reside
- Proficient in Microsoft programs (Excel, Word, etc.)
PREFERRED JOB REQUIREMENTS - Bachelor's Degree Nursing
- CCM Certification
- Current knowledge of CMS (levels of care determinations), appeal rights & DPH regulations
