Job Details

Purpose

To provide short-term clinical social work services to PHA Public Housing residents; serve as an internal source of expertise in mental and behavioral health; advise and coach PHA staff in making referrals to and navigating the behavioral healthcare system. Follow-up on referrals to ensure PHA residents have engaged with a provider for long-term counseling.

Essential Tasks of the Position

Direct Service

  • Processes referrals and conducts intake assessments of individuals and families
  • Diagnose psychological, behavioral and emotional disorders and develop appropriate treatment plans
  • Provides short-term counseling to PHA Public Housing residents, and assists them in connecting them to long-term counseling services
  • Makes appropriate referrals for services, facilitates participant access to services, and tracks status of referrals and the treatment plan for each participant
  • Coordinates a variety of wrap-around services provided to participants
  • Conducts support groups to assist residents to reconnect with their communities and develop healthy coping skills

Collaboration

  • Serves as a member of an interdepartmental team that identifies residents in need of services, develops engagement strategies, and assesses progress in meeting resident needs
  • Contributes to Wellness team by providing behavioral health trauma recovery strategy expertise
  • Outreaches to community-based organizations to build and maintain a strong referral network providing trauma-informed and culturally competent services to program participants
  • Provides and coordinates training to PHA staff about recognizing signs of trauma and trauma-informed practice

Program Management and Development

  • Maintains confidential records on assessments, referrals, and treatment plans
  • Tracks and reports on program and grant outcomes using Resident Services Department database
  • Prepares monthly reports on all program activities and prepares other documents required as part of program oversight
  • Conducts ongoing program development, evaluation, and analysis

Secondary Tasks of the Position

  • Provides trauma awareness and recovery material for PHA newsletters and publications
  • Makes presentations in PHA staff meetings and resident meetings to raise awareness of trauma and coping skills

Position Requirements and Qualifications

Education Level

Master's Degree from an accredited college or university with studies focused on social work, social rehabilitation, psychology or a related field required

Experience in Related Field

  • 5-7 years of case management experience in a behavioral health, medical, social service, or public housing organization
  • 2--3 years of experience with grant delivery and compliance
  • 2--3 years of report writing experience

Knowledge, skills, abilities, licenses and certifications

  • LICSW required
  • Fluency in English and Spanish required
  • Understanding of key issues in trauma and recovery and wellness for diverse families and individuals, elders, and persons with disabilities
  • Familiarity with the network of social services agencies in the community
  • Ability to develop, track and maintain treatment plans
  • Ability to assemble and analyze information for the purpose of developing proposed courses of actions and comprehensive solutions to program-related issues.
  • Ability to work independently and meet deadlines
  • Excellent communication skills (written and oral, including public speaking).
  • Ability to maintain and enforce confidentiality in all assignments
  • Ability to work harmoniously with other agency personnel, consultants, and community agencies and organizations
  • Ability to be flexible and perform work under time pressure
  • Ability to train PHA staff and residents
