Overview:

Central Health is looking for a skilled Licensed Clinical Social Worker to join our Behavioral Health Services (LCSW-BHS) to provide compassionate, inclusive and trauma-informed clinical care with the overall goal to reduce systemic barriers to mental health and substance use supports, treatment and services. In collaboration with patients, their identified support persons, providers, and staff across various disciplines, the LCSW-BHS is responsible for providing strengths-based screening, assessments, diagnostic impressions and treatment to support the delivery of culturally affirming individual, family and group counseling services. The LCSW-BHS is additionally responsible for the delivery of crisis intervention, patient psychoeducation and advocacy, clinical consultation, as well as other relevant clinical services. This position reports directly to the Behavioral Health Manager. Program offerings may be provided in various settings including Central Health's Outpatient Specialty Clinic and other satellite locations. The LCSW-BHS should have an embodied commitment to anti-oppressive and trauma-informed clinical care that fosters patient healing through a collaborative approach centered on empowerment, transformation and hope.



Responsibilities:

Essential Functions:

-Complete screening and comprehensive assessment of the multi-dimensional and holistic biopsychosocial-spiritual needs, strengths, and goals to guide strengths-based treatment planning for diverse breadth of patients.

-Provide trauma-informed, strengths-based and culturally affirming therapeutic counseling for individuals, families and groups to support and enhance a patients overall wellbeing.

-Provide diagnostic impression of mental health and/or substance use disorders utilizing the DSM and deliver effective evidence-informed and emerging best practice clinical treatments.

-Identify and evaluate patient-centered, evidence-informed clinical interventions offered as part of a comprehensive treatment plan developed in partnership with the patient.

-Respond to mental health and substance use related crises through the provision of trauma-informed crisis intervention, de-escalation, risk assessment, and other evidence-informed interventions to patients.

-Advocate with internal and external stakeholders for continued focus on the choices, values and preferences of the patient.

-Work to support safe and seamless continuity of care through the delivery of care coordination services, as needed for patients.

-Actively foster a safe, supportive and healing environment for all patients and their identified support systems.

-Support the Behavioral Health Programs continuous quality improvement and utilization management initiatives and activities to ensure high-quality standards of patient care.

-Attend and participate in scheduled staff meetings, supervision sessions, trainings, and other relevant meetings necessary for the clinical, administrative and operational functions of the Behavioral Health Services program.

-Complete all direct and indirect patient care documentation in accordance with professional standards, program procedures and relevant local, state and federal regulations.

-Maintain knowledge of community resources and other benefits necessary to refer patients for services and to advocate for patient needs, priorities and goals.

-Foster a collaborative and professional environment within the organization and with external community partners.

-Attend relevant professional development training to ensure the delivery of best clinical practices and maintain all licensure and/or certification requirements.

-Perform other duties as assigned.

Knowledge, Skills and Abilities:

-Knowledge of crisis intervention theory and its application in a variety of clinical settings.

-Knowledge of DSM diagnostic criteria for various mental health and substance use disorders.

-Knowledge of the social determinants of health, structural racism and other systemic barriers that contribute to health inequities.

-Knowledge of and adherence to the ethical responsibilities and guidelines set forth by professional licensing boards.

-Ability to provide strengths-based and culturally grounded individual and group services utilizing evidence-informed and emerging best practices centered on addressing each patients individualized needs and goals.

-Ability to assess patient risk of harm to self or others and effectively address any identified risks.

-Ability to be conversant in medical and pharmacological matters, within Social Work scope of practice.

-Ability to collaborate and advocate effectively with internal and external stakeholders to ensure patient goals of care are achieved.

-Ability to effectively engage with patient populations representing diverse social, cultural, and economic histories within a trauma-informed and anti-racist framework.

-Ability to actively pursue the ongoing development of self-awareness as a clinician.

-Ability to enter clinical data into various electronic health record systems while maintaining the integrity, confidentiality and accuracy of the data.

-Ability to facilitate effective and patient-centered screening, assessment, treatment, and discharge planning.

-Ability to provide bilingual clinical services in Spanish, Arabic or Vietnamese, as verified through language proficiency testing, is preferred.



Qualifications:

MINIMUM EDUCATION: Master's degree in social work from an accredited program

MINIMUM EXPERIENCE: At least 1 year of experience performing mental health assessments, diagnosis and individual/group treatment.

REQUIRED CERTIFICATIONS/LICENSURE: Active and unrestricted Licensed Clinical Social Worker in the state of Texas.
