Summary of Responsibilities
JTCHS is seeking individual that will provide Core Medical, Supportive Services such as: Outpatient Services, Health Education/Risk Reduction, Psychosocial Support Service. Care Coordinator will provide data in CareWare; Outreach referring in individuals that need help; Provide Counseling Services; Coordinate care with providers such as psychiatrists, therapists, physicians and nurses; Ensure that all documentation is completed accurately for billing purposes.
Details of Education and Skills:
HS Diploma or equivalent (GED) with minimum 2 years experience working with HIV & LGBTQ population. AA Degree preferred.
Knowledgeable of community resources is essential to the position success.
Experience working with persons in a residential treatment and or in crisis.
Excellent communication ( oral & written) and people skills.
Must be proficient with computers (e.g. Microsoft word)
Special Requirement:
HIV Certificate not required but helpful.
Current CPR (from the American Heart Association).
