General Description:
Provides mental health screening, assessment, brief intervention, crisis intervention and behavioral health management to patients of Evara Health. Works in collaboration with the patient, their family and with the patient s primary care provider, using skills to help patients and their families address psychosocial problems related to and impacting their health and overall well-being. Will help patients reach their patient-centered goals. As a member of a care team, supports Evara Health s mission, vision and goals and are quality focused to enhance patient-centered care.
Essential Duties & Responsibilities
- Performs initial screening, assessments, and evaluation of patients, including but not limited to psychosocial assessments.
- Provides brief interventions and abbreviated therapeutic patient engagement visits
- Assists in facilitating Baker Acts
- Helps patients and families understand how chronic conditions impact their lives and how to address barriers to care including treatment refusal
- Helps patients/families adjust to changes and transitions in life including grief, traumatic experiences, and changes in family dynamics
- Assists patients and families in communicating with one another, and to members of health care team
- Educates patients on levels of health care (e.g., acute, sub- acute, home care); entitlements; community resources; and advance directives
- Plans, arranges, and implements psychosocial programs
- Performs short term counseling and crisis intervention as necessary
- Assists in the implementation of community and social programs in accordance with Evara Health grants/contracts with federal, state, and local agencies and mandates of service requirements with those grants/contracts
- Provides referrals for services both within and outside of the organization that may be needed to address patient conditions
- Uses standardized screening and assessment tools and analysis tools to make objective decisions regarding patient condition
- Coordinates continuity of care planning (including setting follow-up appointments with primary care team), and patient follow-up planning
- Addresses outstanding quality care gaps that impact compliance with optimal management of health and health conditions
- Provides in-service presentations to team members as needed
- Actively participate in assigned Evara Health and community meetings and committees
- Prepare and present patient case reviews as requested
- Acts as a resource for Evara Health employees and community partners
- Responsible for providing top-notch customer service in line with Evara Health policies and procedures, standards, and expectations
- Responsible for accurate and timely documentation within patient records as outlined in workflow and policies and procedures

Minimum Qualifications:
- Minimum of 1 year experience
- Preferred: experience in healthcare setting
- Proficient in general dental practice and medical terminology
- Master s degree accredited by the Accreditation of Counseling and Related programs (e.g., social work, clinical mental health counseling)
- Current, unrestricted Florida Licensed Clinical Social Worker (LCSW)
- Current BLS certification
