Job Description:
We are seeking a dedicated Registered Nurse (RN) to provide skilled nursing care in home health settings. The RN will be expected to deliver comprehensive nursing care, communicate effectively, and work autonomously while maintaining patient confidentiality. A background in medical-surgical and home health care, particularly with wound care experience, is highly desirable.
Primary Duties and Responsibilities:
- Deliver direct or indirect skilled nursing care across various patient groups and diagnoses.
- Implement nursing care plans and interventions, ensuring high-quality preventative, therapeutic, and rehabilitative care.
- Effectively communicate and collaborate with healthcare providers, third parties, patients, and families.
- Maintain patient comfort and safety, observing and recording significant patient conditions and responses.
- Respond to emergencies in accordance with nursing standards and institutional protocols.
- Use electronic medical records and communication tools to document and relay information.
Qualifications:
- Associate degree in Nursing or higher.
- Valid New Jersey RN license and American Heart Association Basic Life Support (AHA BLS) certification required.
- Experience in Med Surg and Home Health; critical care or emergency department experience is a plus.
- Ability to cover the full county as part of the travel radius.
- Proficiency with EMR: Home Care Home Base.
- Bilingual in Spanish preferred.
Schedule and Shift Details:
- Regular hours: 8:30 AM - 4:30 PM, with one weekend per month required.
- Commitment to working one summer and one winter holiday.

Additional Details:
- Unionized position with pay based on experience and potential additional hourly increases for education and certifications.
- Mileage reimbursement at the current IRS rate, with a potential sign-on bonus.
- The role may require travel for 5-7 patient visits per day within Monmouth County.
- Orientation includes 2 full weeks of training, scheduled monthly.
- Equipment provided includes a tablet and phone.
