Description
Community Aging Social Worker needed for full-time, permanent position providing case management and supportive counseling services to older adults to facilitate them "aging in place" in their homes and in the community. Will provide comprehensive case management for assigned older adults including intake, assessment and development of care plans to meet unmet needs.
Will provide support to Case Workers regarding working with clients who present challenging mental health or homeless service needs. They will coordinate care with clients referred by accepting referrals from community business partners and completing assessments and care plans connecting clients to the agency and other community aging service providers.
Annual Salary: $70,000-$72,000.
Position Requirements
Requires LMSW and at least 2 years of social work experience working with older adults or in a mental health setting. Available for occasional evening or weekend events, bilingual strongly preferred.
