Summary

The Clinical Services Supervisor is a professional position responsible for the oversight of the Clinical Services and caregivers performing those services at Advanced Care Partners. This role helps oversee patient care coordination through the supervision of the clinical staff and contractors. The position is also responsible for managing the safety, infection control, and quality assurance program, including oversight of the clinical record review and chart audits and maintenance of medical supplies. Assures medical records are maintained appropriately and that all clinical disciplines function according to licensure, state law, and Medicare/Medicaid standards. The Clinical Services Supervisor reports to and receives consultation, direction, and supervision from the Director of Nursing.

Primary Responsibilities:

Provides direction, supervision, and consultation to the clinical field staff.
Oversees the provision of care to patients in their homes consistent with standards of practice, community need, agency policies and procedures, Medicare/Medicaid guidelines and State licensure laws.
Supervises the provision of clinical nursing care to patients
Works with clinical staff to assist as needed with difficult care issues and to update patient plans of care.
Advises, assists, and collaborates with clinicians on an as-needed basis to assess patient situations, staff problems, and coordinate physician referrals.
Assures that each patient receives quality care consistent with agency policies based on patient need and state/federal guidelines.
Responsible for planning, implementation and evaluation of all performance improvement activities including Outcomes Based Quality Indicators (OBQI).
Oversees proper maintenance of medical/clinical records.
Oversees planning and evaluation of the Infection Control and Safety program for assigned patients.
Completion of all clinical paperwork related to patient care, and authorizations for GAPP and COMP programs
Conducts bi-weekly or monthly supervisory visits and documents appropriately in compliance with all state requirements
Able to be on-call after hours, on weekends within rotation scheduled and as needed basis.
Leads training and orientations for new staff in their region as needed
30 days (about 4 and a half weeks) Conduct employee performance evaluations/appraisals at the probationary period's completion and annually thereafter within 30 days of the evaluation due date.
Supervise direct report caseload outside the patient allocation
Assist HR is ensuring licensure and certifications are up to date
Assess and notify appropriate state regulatory bodies (DFCS, CPS, APS) for any suspicion of abuse or neglect
Complete annual skill competencies and compliance of staff.
Assist Director of Nursing by auditing peer documentation to ensure compliance with company standards and state regulations.
Responsible for ensuring clinical staff within their region are appropriately mentored and supported
Assures accurate and timely completion of field employee incident reports, the handoff to Human Resources, and performs audits of nursing notes for compliance.
Assists in the investigation of patient and customer concerns and complaints, informs the Director of Nursing of patient care and/or staffing issues or personnel issues on a timely basis.
Assures that an adequate stock of medical supplies needed for patient care provision is available for use by staff for patient care within the home.
Assures that Skilled Nurse/CNA/HHA receive supervision, either directly or via other appropriate nursing staff in the home or school setting
Assures effective and efficient utilization of PRN Supervisors in accordance with established productivity guidelines.
Maintains current knowledge of PDN (Private Duty Nursing) principles and practices, applicable laws, regulations, and practices.
Attends external meetings as needed and presents information on Advanced Care Partners Services to outside sources.
Maintains confidentiality of patient and agency information according to HIPAA guidelines.
Performs other duties as assigned.

Clinical caregiver staff turnover
Clinical caregiver call outs or no call/no shows
Ensuring all clinical caregivers receive required orientation/training
Re-hospitalization rates

Requirements:

Current Georgia or compact state registered nurse licensure (RN).
Ability to be a self-starter and demonstrate autonomy
Associate or bachelor's degree in nursing.
Minimum 3 years of experience in nursing, at least one year in home or community health nursing preferable
Excellent time management and communication skills
Minimum 1 year in a supervisory or management position preferred.
Basic computer skills
Valid Driver's license, and car insurance throughout employment.
Fingerprinting is completed in accordance with Medicaid guidelines.
