Job Details
We are seeking a compassionate and skilled Home Health Nurse to fill the role of team supervisor. The ideal candidate will have experience in pediatric nursing, a strong understanding of pediatrics and cerebral palsy, experience in home health, and a history of nurse management (scheduling, chart review, etc.)
Responsibilities
Oversee and manage a team of pediatric home health nurses, ensuring high-quality patient care and adherence to agency policies and procedures.
Develop and implement individualized care plans for each patient, in collaboration with the patient's family, physician, and interdisciplinary team.
Conduct regular supervisory visits to assess the quality of care provided, monitor patient progress, and address any concerns or issues that arise.
Provide clinical guidance, support, and education to staff, fostering their professional growth and development.
Ensure compliance with all applicable federal, state, and local regulations, as well as agency standards and protocols.
Maintain accurate and timely documentation, including patient records, staff schedules, and performance evaluations.
Serve as a liaison between the agency, patients, families, and healthcare providers, facilitating effective communication and coordination of care.
Continuously evaluate and improve the quality of pediatric home health services, implementing evidence-based practices and staying current with industry trends and best practices.
Qualifications
RN Licensure
BLS/First Aid
3+ years experience in pediatrics or home health
2+ years supervisory experience
