The Iowa Primary Care Association is actively recruiting a MAT (Medication Aided Treatment) RN Case Manager for a community health center, also called a federally qualified health center (FQHC), in Sioux City, Iowa.

Position Details:

  • Schedule: 8AM 5 PM
  • In collaboration with provider, performs initial assessment and intake obtaining social, medical, mental health, substance abuse, and medication history
  • Provides education and assessment to patients looking to access treatment programs
  • Offers referral services for patients looking to access treatment programs
  • Assesses and monitors patients in the induction, stabilization, and maintenance phases of treatment
  • Provides ongoing management, education, and support of patients in all phases of treatment including; nursing visits, urine toxicology screens, routine labs, medication teaching, monitoring, pill counts, observed dosing, and medication refills
  • Follow state and federal guidelines in providing care to opioid-dependent patients in collaboration with license, prescribing providers, and pharmacy
  • Complete appropriate documentation for all patient visits and phone calls in a timely manner, ensuring that all notes are complete and easily understood
  • May be required to float to the South Sioux City location once MAT services are offered at that location
  • These functions are not all inclusive

Requirements

  • Minimum of RN license, BSN preferred
  • Current BLS and Mandatory Reporter certifications
  • Clinical office and case management experience preferred
  • Experienced RN with a background in case management who is self-motivated and capable of working with others to improve patient care
