Job Title: Advanced Practice Clinician (Nurse Practitioner/Physician Assistant)


Job Overview: One of our largest clients is looking for an Advanced Practice Clinician (APC) to join their team. We are looking for a provider to join their integrated care delivery program that coordinates clinical care for members in a facility setting. As an APC, you ll provide care to our highest-risk health plan members as part of an interdisciplinary team.

Contract details: 4-to-7-month contract.


Work Schedule: Monday to Friday, 8am to 5pm


Primary Responsibilities:

  • Conduct comprehensive assessments.
  • Manage acute and chronic medical and behavioral conditions effectively.
  • Ensure accurate and complete documentation, including ICD 10 conditions.
  • Collaborate and communicate with the interdisciplinary care team.
  • Conduct advanced illness and advanced care planning conversations.
  • Provide counseling and education to patients and caregivers.


Required Qualifications:

  • Active and unrestricted license in the state of residence, or ability to obtain by start date.
  • Education:
  • NP: Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
  • PA: Graduate of an accredited Physician Assistant degree program.
  • Active Nurse Practitioner or Physician Assistant certification through a national board.
  • Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations.
  • Access to reliable transportation
  • Ability to gain a collaborative practice agreement.


Preferred Qualifications:

  • 1+ year of clinical experience in practice (long-term care setting preferred).
  • Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs.
  • Understanding of Geriatrics and Chronic Illness.
  • Understanding of Advanced Illness and end of life discussions.
  • Proficient computer skills including the ability to document medical information with written and electronic medical records.
