Advocate/Case Manager

Status: Full-Time HourlyFLSA Classification: Non-Exempt

Summary of the Position: The primary responsibility of the Case Manager/Advocate is to initiate, organized, plan, develop, and implement direct case management and advocacy services to assigned clients and their families as part of the Merrimack County Intensive Service Option (ISO) In-Home program.

  • All service plans are based on strength-based approach using the wrap around model.
  • The Advocate maintains a caseload of no more than 10 families per month on average and is required to provide a minimum number of face-to-face hours with families each week. Documentation must be completed in accordance with New Hampshire Medicaid standards.
  • Applicant must be dependable, committed and able to serve as a positive role model for youth in community, school, and home settings.
  • The position offers flexible hours, competitive weekly pay and activity reimbursement.


Qualifications/Requirements: Bachelor's degree in social work, psychology, education or a related field with an emphasis in human services and two years of experience working with children and families.

  • Demonstrated communication skills, both oral and written. Experience in the provision of home and community-based services.
  • Position requires reliable transportation, valid driver's license, and car insurance limits of $100,000/$300,000 bodily injury liability.
