We need a full-time STNA Social Services Designee to join our team in Greater Cincinnati & Northern, KY.

You ll assisted our Licensed social Worker by ensuring that the medically-related emotional and social needs of our residents are individually met and maintained.

$15-$18.50 Per Hour

You ll Be Responsible For:

• Assisting with gathering information from residents and family members
• Assisting with the completion of the Integrated Personal Services Assessment and MDS
• Assisting with the discharge planning process by arranging community resources, alternative placement, etc
• Attending Care Conferences as directed by the LSW
• Assisting LSW with any other case management tasks deemed necessary to the Department
• Fulfilling basic documentation requirements for social services
• Assisting (after training) in the BIMS and the PHQ-9
  
  

You ll Need To Have:

• An STNA certification or eligibility and experience in long term care
• Excellent communication skills
• A High School Diploma and/or GED
• Healthcare industry, social work, or other related experience
• The ability to multi-task in a fast-paced, high volume environment
• Exceptional organization skills with strong attention to detail
• A personable and professional demeanor
• Computer proficiency in Excel, Word, Power Point, and Outlook
