Methodist Fremont Health is hiring for a Social Worker in Home Health & Hospice!


As a vital member of the Methodist Fremont Health team, the Social Worker in Home Health & Hospice plays a pivotal role in patient care, collaborating closely with physicians and interdisciplinary teams to elevate clinical outcomes and ensure patient satisfaction. In this dynamic position, you'll be responsible for crafting and executing discharge plans tailored to the unique needs of complex and at-risk patient populations. Your expertise will shine as you intervene on behalf of patients with intricate psychosocial requirements, navigating eligibility for social programs and funding sources with compassion and precision. Job Duties & Responsibilities:

  • Develops and implements complex patient discharge plans.
  • Facilitates referrals to post-acute agencies and assists with advance directives.
  • Collaborates with multidisciplinary team on discharge planning.
  • Engages with diverse patient populations and conducts interventions.
  • Provides counseling to optimize health outcomes.
  • Documents patient interactions and participates in quality improvement activities.
  • Ensures adherence to safety protocols and utilizes effective communication principles.


Schedule:

  • This is a full time 32hr/week position, hours will vary


Location:

  • Methodist Fremont Health Hospital- 450 E 23rd St, Fremont, NE


Salary:

  • $24.20-$38.70, based on work history/experience


Job Qualifications:

  • Bachelor s degree or equivalent qualification (e.g., CPA, PE, etc.).
  • Bachelor's degree in Social Work (BSW) from a Council on Social Work Education accredited school required.
  • Pursuit of Master's degree in Social Work preferred.
  • 0-1 years of social work experience required.
  • 1-2 years of medical social work experience preferred.
  • Experience in Home Healthcare and Hospice preferred.
  • Certification as a Social Worker (CSW) required.
  • Preferred Nebraska license as a Certified Bachelor of Social Work (CBSW).
