GENERAL SUMMARY

Under direction, provides crisis intervention and de-escalation services in the home and community. Completes crisis intervention (face to face) to de-escalate the individual in crisis and determine if the individual is in need of additional support/interventions. Maintains clinical and administrative records. Coordinates, consults and provides liaison work with other agencies and systems. Implements program goals and activities in keeping with program philosophy.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

Adult Mobile Crisis therapist will utilize de-escalation techniques, stabilize the crisis/current situation, separate the individual as appropriate, identify strengths of the individual and provide appropriate therapeutic intervention.

Conducts thorough, accurate, and clinically sound triage/screening with consumer/guardian/family members in a timely manner.

Performs direct clinical services, including screening of referrals, assessments, crisis intervention and de-escalation for adults. Provide evidence-based therapy services to individuals served in the crisis stabilization program.

Responsible for crisis intervention, treatment plans, case management, and transition services.

Provides case management services for consumers to help obtain necessary services and coordinates them until they are transitioned to a service provider.

Coordinate treatment with service providers if the individual is in treatment.

Provides follow-up services for individuals served to help obtain necessary services, coordinating with Macomb County Community Mental (MCCMH) Managed Care or current provider to ensure the consumer is receiving the proper level of care.

Maintains clinical documentation to ensure accurate, timely and complete assessments, progress notes, crisis/safety plans are following Macomb County Community Mental Health (MCCMH) policies and procedures.

Requests appropriate authorizations for services provided; monitors utilization of services; requests re-authorizations or continuations as appropriate. Serves on community mental health or community-based committees as directed.

Ensures administrative paperwork is accurate and up to date, including staff activity logs, statistical reports, attendance, mileage reports, and case lists.

Participates in agency and system staff meetings.

May supervise the work of department interns.

Performs other duties as assigned.


QUALIFICATIONS

Required Education and Experience

  • Master s Degree in Social Work, Counseling, Psychology or Doctorate in Psychology or a directly related mental health field from an accredited college or university
  • Two (2) years of clinical experience working with adults with Serious Mental Illness


Preferred Education and Experience

  • One (1) year or more of experience working in a community mental health setting
  • Two (2) years clinical experience working in crisis-oriented services

Required Licenses or Certifications

  • Licensed Master Social Worker (LMSW), Limited Licensed Master of Social Work (LLMSW), Licensed Professional Counselor (LPC), Limited License Professional Counselor (LLPC), Temporary Limited Licensed Psychologist (TLLP), or Licensed Psychologist (LP)
  • Possession of required state licensure appropriate to position and ability to meet privileging and credentialing guidelines
  • Possession of a valid Michigan driver s license and operable, insured automobile for authorized travel

Knowledge of:

  • Working knowledge of resources within MCCMH and outside of Macomb County Community Mental Health
  • Considerable knowledge of the Michigan Mental Health Code/401 criteria
  • Knowledge of mental illness, serious emotional disturbance, and developmental disabilities
  • Strong knowledge of techniques to de-escalate/interact in potentially dangerous situations
  • Knowledge of applicable computer applications

WORK ENVIRONMENT/CONDITIONS

Works in an office environment and uses a computer, telephone and other office equipment as needed, to perform duties. The noise level in the work environment is typical of that of an office with frequent contact with customers.

Bends, stoops and reaches in order to file, search for and retrieve records and documents.

Manual dexterity and regular fine-finger and hand/wrist motions are required for operating a keyboard, writing and filing.

Seeing (vision), sitting, talking/speaking, or listening/hearing are regularly required.

Operates an automobile while performing assigned job duties.
