**Responsibilities**

The Intake Counselor/shift leader provides telephonic and in-person risk assessment and crisis intervention, initial contact with referral sources and potential patients, coordinates the referral and intake process, determines appropriate level of care, facilitates the admission process and maintains communication with referral sources, families, patients and unit staff.

Although many duties will be the same for all shift leaders, certain qualities/abilities may be more important in specific shifts.

The evening leader must demonstrate the ability to operate with more autonomy as day staff leaves hospital, ability to process and problem solve multiple transfer request issues, and work cooperatively with the nursing supervisor and unit staff to provide quality care to our patients.

**JOB SPECIFICATIONS:**

Staff must demonstrate knowledge and skill in provision of services to varied population of patients in an inpatient and partial hospital setting.

- Represent the standards of Service Excellence: Treating people as guests, acting professional in what you do and say, teamwork.

- Receive hand-off from overnight shift and follow up with pending transfer requests/calls/precerts

- First point person for staff questions regarding processes and clinical

- Run call log report and revise if needed. Complete deflections.

- Make copies of report for Flash meeting

- Update precert board and follow up with cases that are pending

- Reconcile Intake and Unit boards/update census

- Review room alones with house supervisor/MD

- Prioritize department tasks and assign

- Problem solve bed, referral source and patient needs

- Collaborate with nursing supervisors specifically around transfer requests and transition of patients from Intake to Units

- Collect data for PI

- Check precert line throughout shift

- Assist in interviewing and training new staff

**Pay Range:** $27.50-$35.00/hr, eligible for additional shift differentials

**Qualifications**

**Education:** Masters degree in Counseling, Psychology, Social Work or Human Services field.

**Experience:** Prior intake, assessment, admission and crisis intervention experience with a psychiatric mental health population, preferably in an inpatient setting.

**Licensure/Certification:** LCSW, LPC, LMFT, RN preferred

**Knowledge:** Prefer knowledge of crisis intervention and risk assessment, psychiatric diagnoses symptomology, basic chemical dependency, age specific growth and development and crisis and behavior management.

**Supervision Received:** Intake Director

**Supervision Exercised:** N/A

**Environmental Conditions:**

Works in general office environment and hospital units/programs. Exposure to psychiatric patients who may exhibit violent/aggressive behavior. Potential for exposure to communicable diseases, blood/body fluids and other hazardous waste. General hospital environment.

**Physical Requirements:**

Ability to communicate verbally, telephonically and in writing. Physical agility to manage patients. Push, pull and lift up to 40 pounds. Intermittent sitting, standing, walking, bending and stooping. Drug and alcohol free as evidenced by negative results of post-offer, pre-employment urine drug screen.
