Position Description: Home Care Aide

Status: Part-Time With Benefits: 24 - 29 Hours per Week. Potential of moving to full-time.

Shift: Day - 8:00 a.m. to 4:30 p.m.

Days: Monday thru Friday; including 1 weekend per month

Position Summary: Provides personal care and supportive services to home care clients in their place of residence to promote rehabilitation, safety, and independence within the individual's physical and mental limitations.

Education/Qualifications: High School Diploma or equivalent; Must have 75 Hour CNA training and certification. Requires current driver's license and auto insurance, with the ability to travel throughout Boone County.

Essential Functions:

  • Regular attendance
  • Excellent verbal and written communication skills
  • Possess or have access to reliable transportation for all of Iowa's seasons
  • Ability to think and act independently when required
  • Confidence in providing personal cares and assisting with ADL's
  • Knowledge of computer and mobile devices for training and documentation purposes
  • Excellent organization and time management skills
  • Consistency in keeping on schedule and attending appointments on time according to home care's policies and procedures
  • Demonstrates knowledge and skills necessary toprovide care appropriate to the ages of the patients served within thedepartment
  • Maintains performance in compliance with Stateand Federal laws, and consistent with high standards of business and professionalethics
  • Provides personal care assistance to patientssuch as bathing, grooming, dressing, simple wound cares, ambulating, andrehabilitation
  • Assists patients with oral medications asdirected by home care professional
  • Provides meal planning and preparation asdirected by supervisory staff
  • Provides basic light housekeeping such as sweeping,mopping, vacuuming, dusting, cleaning bathroom/kitchen areas, changing linens,and laundry
  • Provides miscellaneous duties such as grocery shopping,errands, money management/bill paying, and parenting skills
  • Documents services provided according todocumentation standards of the department
  • Reports changes in patient condition tosupervisor
  • Maintains effective communication with alllevels of supervision and staff
  • Maintains HCA scheduling planner, and attendsregular scheduling meetings
  • Attends and participates in department staffmeetings/in-services and hospital wide mandatory in-services
