Health Home Care Coordinator

Division:Comprehensive Care FTE1.00 Summary Core Responsibilities

* Participate in the on-call process.

* Demonstrate effective leadership in managing daily operations of in home services.

* Perform initial assessments of participants in their home environment to determine their individual needs and coordinate plan of care with care-giving resources. Coordinate with the interdisciplinary team to develop care plan for each participant.

* Oversee or perform annual re-assessments of all participants to determine that appropriate in home services are provided and that participants are in the least restrictive environment.

* Function as a member of the interdisciplinary team. Maintain regular attendance at, and participate in interdisciplinary team meetings, communicating participant changes, collaborating on care planning decisions and coordination of 24-hour care delivery.

* In partnership with Home Health API community based agencies, provide orientation, initial and annual competencies, and evaluates performance of Home Health Aides and Individual Providers.

* Perform competencies on appropriate Home Health staff as needed.

* Perform quarterly supervisory visits to staff in the field as appropriate

* Ensure that In-Home staff provide skilled supportive and restorative nursing care to participants as ordered, based on the plan of care

* Ensure staff maintain participants medical records and fulfill charting and reporting requirements

* Responsible for the coordination and scheduling of personal care activities and services of all participants in the home.

* Serve as a liaison between PACE and contracted Home Health services not limited to; Evaluation of services provided by contractor; Coordination of services when one or more licensed agencies are providing care to the patient; All direct care personnel, contractors, and volunteers initiate emergency procedures according to agency policy

* Manage and prioritize utilization of homecare staffing needs to reflect actual enrollee homecare needs. Provide this information to the interdisciplinary team.

* Ensure all contracted Home Health agency staff complete initial and annual competency testing, and fulfill all CMS and PACE training and personnel requirements.

* Act as a Home Health administrator as defined by WAC 246-335-515 section 11; oversee the day-to-day operations and fiscal affairs of the Home Health program

* Act as the Home Health director of clinical operations as defined by WAC 246-335-545

* Maintain fiscal accountability for home care services provided.

* Maintain the confidentiality of all company procedures and information about participants, clients or families.

* Participate in continuing education classes and any required staff and training meetings. Maintain professional affiliations and any required certifications.

* Act only within the scope of his or her authority to practice.

* Practices universal precautions and follows appropriate Infection Control procedures.

* Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.

* Must ensure: (a) Coordination, development, and revision of written patient care policies and procedures related to each service provided

Position Requirements **Education** **-** Bachelors degree preferred but not required.

**Experience** - 1 year of experience with homecare and home assessments preferred and a minimum 1-year experience working with a frail or elderly population.

**Other Requirement(s) -** Valid WA State Drivers License and proof of auto insurance. BCLS Certification preferred but not required
