Description

The LIFE PACE (Program of All-Inclusive Care for the Elderly) Clinic team performs a vital service by providing primary care for Tulsa's aging population. The goal of our amazing team of professionals is to provide outstanding services and oversight in order to keep participants in their homes for as long as safely possible in lieu of nursing facility placement.

We are looking for a Social Services Coordinator to grow our staff and join a team of professionals, who excel in providing unique health care for Seniors! The Program of All-Inclusive Care for the Elderly (PACE) offers older adults and their caregivers a welcome alternative to nursing home care. Participants have access to comprehensive medical services and social support at the site while they enjoy independent living in their own homes and communities.

In this hands-on role, you will be assisting the Social Workers with participants in need with information, assistance and referrals to community-based services. Partnering with the Social Worker and utilizing compassionate active listening, assessing the needs of the participant and offer information and/or referrals to services that meet their needs. When appropriate, engaging in more extensive assistance by serving as an advocate for our participant when questions and problems arise. This includes, completing applications at a participant's home and/or meeting participants at sites. Sites, such as, social security office, tag agency, phone store or apartments. Since this position will be working exclusively with seniors, some with dementia, patience and extra compassion will be required.

Examples of such issues are:

Housing applications and research of available apartments

Assistance with requesting birth certificates

Assistance going to tag agency for ID Assistance

Going with the participant to get a government cell phone

Calling social security with participants to request benefit letter

Locating resources for needs such as rent assistance, utility assistance, home repairs - then completing necessary application for these resources

Requirements

EDUCATION AND EXPERIENCE:

Bachelor in Social Work (BSW) is required.

Prefer minimum of one (1) year of documented experience working with an elderly population.

Cell phone, reliable transportation, verifiable car insurance and good driving record.

Experience with researching and connecting to community resources for our participants.

Excellent communication and the ability to multi task is a plus.

Bilingual Spanish is a plus.
