Purpose of this position

The purpose of this position is to provide direct care to residents, under the supervision of a licensed nurse, in accordance with facility policies and procedures and report resident needs and concerns to a licensed nurse.

Qualifications

  • Must have current license
  • License must be free of any encumbrances
  • Must not have any disqualifying findings barring employment as stated by State or Federal regulations

Delegation of authority

  • Provide care to residents following facility policies and procedures
  • Report residents' needs and concerns to a licensed nurse

Primary Functions and Responsibilities

  1. Follow written and oral directions
  2. Follow established performance standards and perform duties according to nursing service policies and procedures
  3. Assist new employees in following established facility policies and procedures
  4. Identify resident problems and concerns and report them immediately to a charge nurse or a licensed nurse
  5. Complete assignments timely, completely and accurately
  6. Conduct resident rounds as assigned
  7. Initiate corrective action as necessary and/or seek assistance of a licensed nurse
  8. Identify safety hazards and emergency situations, and initiate corrective action
  9. Attend all in-service classes as assigned and complete assignments
  10. Assume personal responsibility for following facility procedures related to control of equipment and supplies within the unit
  11. Document incidents and/or unusual problems according to established facility procedures
  12. Participate in interdisciplinary team and other facility meetings as assigned
  13. Participate in the development of an individualized plan of care of assigned residents
  14. Review care plans and perform nursing care as outlined
  15. Document in the Point of Care Kiosk the care and treatment provided to the resident and the resident's response or lack of response to care provided. Documentation is to be completed at the time care is given and all documentation must be finalized at the end of the assigned shift
  16. Maintain confidentiality of resident and facility information
  17. Report changes in residents' conditions to Charge Nurse or Nurse Supervisor
  18. Follow facility procedures for admission, discharge and transfer of residents
  19. Complete documentation accurately, timely and following facility policies and procedures
  20. Listen to resident and family complaints and report problems to Charge Nurse
  21. Assure residents have call lights at hand and answer call lights promptly
  22. Assure residents are given adequate assistance with personal and oral hygiene needs following facility's policies and procedures
  23. Assist residents with activities of daily living as documented in the residents' care plans
  24. Participate in the implementation of the individualized plan of care of assigned residents
  25. Assure residents are given adequate assistance with meals, nourishment and transportation to meal service area(s)
  26. Ensure that all residents that have identification devices are present and functioning
  27. Consistently work cooperatively with residents, charge nurses, treatment nurses, restorative nurses, other nursing assistants, physicians, families, consultant personnel and ancillary service providers
  28. Observe all facility safety policies and procedures
  29. Come to work in a clean, neat uniform and consistently present an appropriate professional appearance
  30. Come to work as scheduled and consistently demonstrate dependability and punctuality
  31. Assume accountability for data contained in the employees' handbook
  32. Perform other related duties as directed by his/her supervisor
  33. Perform all duties assigned in an effective, timely and professional manner
  34. Assume Quality Assurance duties as assigned
  35. Observe infection control procedures
  36. Follow Residents' Rights policies at all times
  37. Be responsible for the safety of residents under his/her supervision
  38. Ensure that residents Right to Privacy and Dignity are observed at all times
  39. Accept assigned duties in a cooperative manner
  40. Has completed the state-approved Nursing Assistant Training Program
  41. Completes yearly 12 hours in-service programs as designated by federal government
  42. Attends annual training in dementia management and patient abuse prevention
  43. Uses only authorized abbreviations established by the facility when recording information
  44. Agree not to disclose assigned user I.D. code and password for accessing resident/facility information and promptly report suspected or known violations of such disclosure to the Charge Nurse
  45. Agree not to disclose resident's protected health information and promptly report suspected or known violations of such disclosure to the Charge Nurse
  46. Maintains professionalism at all times, does not discuss personal problems with residents and ensures that communication while providing bed-side care is appropriate and beneficial to the resident
  47. Ability to work other shifts as needed
  48. Complete all duties assigned by his/her supervisor
