Mount Alverna Village is looking for a Director of Social Services!

Full-Time, Exempt

Mount Alverna Village, a part of Franciscan Ministries, is a vibrant senior living community in Parma, Ohio. We are a continuing care retirement community (CCRC), offering a variety of senior lifestyle options including independent living, assisted living, assisted living memory care, and skilled nursing/long-term care. Additionally, the onsite rehabilitation clinic offers inpatient and outpatient physical, occupational, and speech therapies, and state-of-the-art programs designed to enhance the quality of life for residents and patients in the local community.

The Director of Social Services is essentially responsible for the planning, organizing, development, and direction of the overall operations of the Social Services Department in accordance with current federal, state, and local standards, guidelines, and regulations, and facility policies and procedures to assure that the medically related and emotional and social needs of the residents are not met/maintained on an individual basis.

Specific Tasks/Duties:

  • Assessments
    • Assesses residents, including mood, behavior and psychosocial issues, family relationships, coping mechanism, reactions to institutional placement, and assessment of level of care required
    • Works in collaboration with physicians and other health care personnel in resident evaluation and treatment and discharge
    • Participate in resident assessment and implement social service plan of care
    • Encourages Residents to participate in the development and review of own plan of care as appropriate
    • Participate in resident care conferences and family meetings as needed

  • Plans, develops, organizes, implements, evaluates and directs the social work program of the facility. Develops and implements policies and procedures for the identification of medically related social and emotional needs of the resident.

  • Budget/Fiscal Accountability Participate in budget preparation process related to department. Ensure department hours of labor including overtime and departmental expense management are performed to ensure fiscal management of the department and compliance with budgetary targets

  • Attends and actively participates in staff meetings, department managers meetings, designated committee meetings (such as ethics, QAPI, Ethics, U.R., Grievance).

  • Works with the Admissions department as needed to coordinate new admissions, room changes and discharges.

  • Responsible for overseeing the Discharge Planning Process.

Qualifications/Requirements

  • Minimum requirement of a Bachelors Degree in Social Work, preferably a Masters degree in Social Work.
  • Long term care, MDS, PPS and care planning. Minimum of 2 years in Social Service Field
  • Licensed Social Worker
