What you'll do as a Medical Social Worker (MSW) Case Manager:

Conducting comprehensive psychosocial assessments of patients and their families to identify their needs, strengths, and resources
  • Developing and implementing care plans that address the psychosocial needs of patients and their families
  • Providing counseling and emotional support to patients and their families, including grief and loss counseling
  • Coordinating community resources and services to meet the needs of patients and their families
  • Serving as an advocate for patients and their families, helping them to navigate the healthcare system and access needed resources
  • Participating in interdisciplinary team meetings to develop and review patient care plans
  • Maintaining accurate and timely documentation of all services provided


Requirements for Medical Social Worker (MSW) Case Manager:
  • Master's degree in Social Work from an accredited school of social work
  • Current state licensure as a Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW)
  • Minimum of 2 years of experience working in hospice or palliative care
  • Excellent communication and interpersonal skills
  • Ability to work effectively as part of an interdisciplinary team
  • Knowledge of community resources and services that can support the needs of hospice patients and their families
  • Ability to provide compassionate and empathetic care to patients and their families
