We re looking for motivated licensed social workers who are passionate about discharge planning and connecting patients to community resources!
Overview:
- The Inpatient Social Worker acts as a coordinator of psycho-social assessment and discharge planning for the patients/clients receiving inpatient services in the hospital setting.
- Clinical supervision hours are accrued!
- Direct hire role.
Responsibilities:
- Acts as a coordinator of psychosocial assessment and discharge planning for the patients/clients in a Hospital setting
- Providing psychosocial support necessary to address the individual's needs.
- Acting as a resource to patient/client, utilizes knowledge of community and health care resources to assist in the addressing identified needs at any point of the continuum., pre-admission to post-discharge.
- Using timeliness and appropriateness of intervention, facilitates quality resource utilization. i.e. Length of stay and level of care.
Requirements:
- LMSW or LCSW required
- Fully vaccinated and boosted for COVID
Specifics:
- Monday- Friday onsite : 8:30AM - 5PM (Full time hours only)
- Location: Baltimore, MD
- Salary: $56,000 - $88,000
