Job Purpose & Position Overview

The purpose of this position is to provide contact and referral for general medical patients and residents of the Winnebago community. The CHR Generalist may be required to assist in developing routine programs aimed at benefitting the greatest number of community members as well as provide weekly projections for work schedule and prepare/maintain progress reports on all program involvement of clientele.

Specific Job Duties/Responsibilities:

HOME VISIT INITIATIVE

  • Home visit assigned clients as scheduled by DPHN and CHR Coordinator.
  • Assist with medication needs and translation.
  • Provide transportation to and from PHS Hospital and Tribal Health Programs.
  • Provide supervised home health care for assigned clients, and will include:
  • Blood pressure/blood glucose monitoring.
  • Provide medication education.
  • Monitor wound care dressing changes to wounds Stage 1 per orders.
  • Educate on healthy lifestyle habits.
  • Refer to resources as necessary

  • Maintain records and reports as necessary to supervisor and DPHN.
  • Follow-up on at-risk community members.
  • Notify patients of scheduled doctor appointments.
  • Follow-up from PHN and referred provider appointments.
  • Assist with infections disease screenings.
  • Attend all in-service and classes that are health related when possible.
  • Attend mandatory training by the Tribe and HIS.
  • Prepare monthly reports and submit to supervisor.
  • Complete patient charting (CHRPCC data reports) in a timely manner.

EMS DUTIES
  • Respond to ambulance calls during designated shifts and assist with coverage of available work shifts on a 12 hour rotating basis as well as provide coverage for on-call shifts.
  • Responsible for providing emergency transport and treatment of sick and injured patients of the Winnebago community and surrounding areas.
  • Attend all EMS related training courses.
  • Ability to immediately respond to high emotion/high stress or physically taxing situations without prior warning and the ability to warm up to and quickly return to daily activities thereafter.
  • Responsible for providing safe motor vehicle operations during transportation of patients for the duration of the emergency call.
  • Responsible for the safety and security of patients at all times.
  • Must be punctual for shift assignments to provide relief for coworkers scheduled on previous shift.

SPECIALTY AREA
  • The position is general in nature and will work with other CHR staff and health programs
  • Attend all health and injury prevention related in-services, workshops, and training courses as assigned.


PERFORM ALL OTHER DUTIES AS ASSIGNED BY CHR COORDINATOR
  • Deliver commodities to the homebound and Senior Citizens,
  • Respond to CHR calls after hours within an assigned on-call period.
  • Responsible for maintaining GSA vehicles at all times.


Qualifications

Required Qualifications/Skills:
  • High School Diploma or GED is required.
  • Must possess a valid driver's license and be able to meet driver eligibility guidelines.
  • Must be in good physical and mental health and pass a complete physical exam prior to employment (to include PPD).
  • Must have and maintain a home telephone.
  • Must have basic knowledge of MS Office computer software.
  • Must possess the ability to maintain accurate written daily, weekly, and monthly reports.
  • Must perform all duties in a manner that ensures the strictest confidentiality due to access to sensitive information.
  • Must have and maintain CPR/BLS certification.

Training Requirements:
  • Must attend all department meetings, trainings, and certification courses as required by position and CHR Coordinator.


Physical Requirements:
  • Normal to heavy work which includes the ability to lift up to 75 lbs.
  • Regularly required to walk, stand, and sit for extended periods of time and use hands to operate computer keyboard, phone, various office equipment, and GSA vehicles.
  • While performing daily duties, the employee is regularly required to talk and hear.

Certificates, Licenses & Registrations:
  • All applicants will comply with 45 CFR 1301, Subpart D, Section 1301.31, C and D, which require all prospective employees to sign a declaration prior to employment regarding all arrests and convictions of child abuse or violent felonies and to comply with PL 101-630 and PL 101-637 regarding criminal records check.
